NZ LCP Goal Data Dictionaries for versions 11 and 12

Recording the outcome of goals is crucial. The goals on the LCP and the associated coding and recording of variance needs to be fully understood if the LCP is to be fully implemented and sustained within the clinical environment.

Please note there are now two Goal Definition - data Dictionaries - one for NZ LCP version 11 documents and one for NZ LCP generic Version 12. Each has been adapted to reflect the goals of care in numerical order as they appear in either version. NZ LCP generic version 12 is the most up-to-date version of the LCP. When adapting the LCP locally, the "goals of care" in the LCP must remain the same. The National LCP Office NZ encourages registered LCP sites/organizations to utilize the appropriate NZ Goal Data Dictionary for the LCP version you are using to check that the addition of local prompts under the "goals of care" have not changed the meaning of the goal. All adapted Versionn 11 and version 12 NZ LCP documents should be forwarded to the National LCP Office NZ for review to ensure compliance before being used in practice.
